*Please note: To qualify for this registration fee, proof of status (student, fellow, resident, etc.)  must be submitted at time of registration – i.e. a signed letter from a supervisor or professor or a copy of student ID. This must be forwarded to the Congress Organizer at the time of registration via fax at (703) 783-0501 or email at iciem2009@jrdaggett.com. 

**Please note: To qualify for this registration fee, you be an official representative from a nonprofit, or family/patient advocacy group.   

***Please note:  Accompanying persons are spouses and/or other social guests (age 12+ only) of fully registered delegates.  Professional delegates may not register as guests.
